A summary of the exchange on breast oncology between Fiorentini and Notter:

Breast cancer classification and clinical grouping has been subject to a continuous change in the past decades. Based on extent and morphology with advent of gene expression analyses, we consider the disease a heterogeneous entity. Histologically the majority is classified as invasive carcinoma of no special type (formerly known as invasive ductal carcinoma), smaller proportion belongs to invasive lobular carcinoma and even less cases represent rare histotypes.

Phenotypically, applying immunohistochemistry in routine, we can delineate hormone receptor positive and negative groups which can further be divided into Luminal A, Luminal B, HER2-enriched, and basal-like breast cancer identified originally on gene expression arrays. Clinically we also incorporate stage and extent of tumor with TNM classification, and the line of treatment into our consideration and indication of therapies. All the above with characteristics of the patient itself are cornerstones of administration of oncotherapies.

Fiorentini and Notter shed light on this multi-dimensional thinking in breast oncology and sharply point out directions towards an improved care. With all parameters we can conclude on the best practices, design of appropriate trials and draw the conclusions to treat our breast cancer patients better. There is still a long way to go – despite the fact, that breast cancer is probably the most investigated cancer type among all solid tumors – and we should improve efficacy of current standards and build up the therapy of the future.
